Pigmentations on the body are usually caused by excessive melanin. Laser provides an excellent way to treat some of the common pigmentations of the face. Prior to treating it is very important to rule out malignant melanoma.
Melasma (also known as "Chloasma faciei" or the mask of pregnancy when present in pregnant women) is a tan or dark skin discoloration. Although it can affect anyone, melasma is particularly common in women, especially pregnant women and those who are taking oral or patch contraceptives or hormone replacement therapy (HRT) medications. It is also prevalent in men and women of Native American descent (on the forearms) and in men and women of German/Russian and Jewish descent (on the face).
The symptoms of melasma are dark, irregular patches commonly found on the upper cheek, nose, lips, upper lip, and forehead. These patches often develop gradually over time. Melasma does not cause any other symptoms beyond the cosmetic discoloration.
The discoloration usually disappears spontaneously over a
period of several months after giving birth or stopping the oral contraceptives
or hormone replacement therapy.
Treatments to hasten the fading of the discolored patches include medical treatment and laser treatment.
The treatment of melasma had been difficult until the invention of Fraxel machine for fractional resurfacing of the skin. By combining this with other forms of medical treatments optimum results can be obtained.
Fraxel is also used for treatment of senile spots and fine lines. It has been very useful in the treatment of acne scars. Acne scars require the treatment to be repeated many times until the satisfactory results are obtained.
Our treatment of pigmented patches combine the IPL ( Intense Pulse Light) and RF (Radio Frequency) which is combined with Fraxel (erbium) laser to resurface areas with very dark pigmentation. The IPL with RF takes out the pigmentation from the superficial and middle parts of the skin. The fraxel resurfacing is the best way to remove deep pigmentation in asian skin. The erbium laser does very little damage to the epidermis compared to the C02 laser. Asian skin is at higher risk of developing post inflammatory hyperpigmentation and therefore the newer C02 laser is not recommended. The same results can be achieved with less risk with the Erbium laser.

Fraxel is also great to treat the age spots of the hands. It requires a few treatments each one a month apart.
Fraxel is also great for treatment of crows feet lines on the face. These are the fine wrinkles which appear from ageing lateral to the eyes.

as improvement of the pigmentation of the skin achieved wth Matrix IR treatment.
Matrix IR is also fractional laser technology. The Matrix IR applicator uses a
proprietary synergy of light and conducted Bi-Polar RF to create the deep
thermal heating needed to both shrink and generate new collagen while protecting
The Matrix IR combines a diode (915 nm) laser matrix, bi-polar conducted radio frequency energy and intense epidermal cooling. Use of this combination of modalities results in overlapping light beams penetrating deep into the dermis to a depth of up to 2.5 mm to pre-heat the target tissue, and the RF energy being drawn to the target. This puts the thermal zone where it is effective for wrinkle treatment, while being gentle on the epidermis.
This treatment works for fine wrinkles as well as pigmentations. In some studies 80% results were obtained with Matrix IR in cases of melasma. I have had great results with skin pigmentation by using Matrix IR. For the best results the laser can be combined with skin creams.
If the pigmentation is superficial the best option for treatment is with photofacial treatment. The best photofacial treatment for the common type of skin in Sri Lanka is offered by Syneron ELOS technology which combines photofacial therapy with radiofrequency.

With this techonology the laser which targets a certain
tissue is applied to the skin. The laser heats up certain desired tissue
depending on the wavelength of the laser. Then the electrical energy is applied.
Electrical energy is safe for all types of skin. It goes through the heated
tissue thereby providing a better result. In our clinic we use ELOS technology
for photofacial, varicose vein treatments, hair removal, skin tightening and
cellulite reduction therapy.
Above shows remarkable improvement with combinatino of Fraxel and ELOS technique combined with the unique Lightening skin cream which is only sold in our clinic. Only two treatments were given at the time of the second picture taken without any makeup. Patient is already very happy with the results and will continue to receive further treatments for removing the remaining pigmentation.
Fraxel is also used to remove acne scars. Acne scars require about six monthly treatments.
Above is a patient who became fairer and got rid of most of her pigmented patches of the face with just three treatments with IPL RF and Fraxel. She also used our specially formulated day cream and the night cream to achieve the above result.

Once you have obtained a good result it is very important to maintain that result with the same treatment once in four or six months. Otherwise the skin tends to revert back to where it was gradually.
In addition to taking out pigmentation from the skin fraxel resurfacing can be used to treat acne scars. The above results took six fraxel treatments done once a month. The depth of the fraxel laser is usually increased gradually.
